http://www.ehbfree.com/signup and complete the request form. Upon submitting, your request will be parsed into our Account Requests queue and an automated reply confirming your request will be sent to your e-mail.
Please submit only one request and wait patiently for a response. Submitting multiple requests will result in ALL requests being denied. An Accounts Representative will review your application within 24 hours, and reply to your request stating if you were approved or denied.
