I'd like a reliable host for a mainly php website. I don't want it to go down 1 or 2 months after I sign up. My requirements are:

- PHP
- At least 2 SQL databases
- Around 100mb of storage
- At least 200mb of bandwidth
- Instant setup with no forum posting required
- I would preferr no ads, but if necessary I will endure them
- Ability to host .exe and .zip files
